Abstract
The numerical evaluation of kidney function has been considered with greater significance in clinical practice. The authors employed a clearance method with endogenous creatinine, observed renal effect of oral diuretics, Diamox, in normal adults, measuring glomerular filtration rate, water reabsorption rate and urine pH chronologically.
Materials and Method
The subjects were allowed ordinary breakfast, containing not more than 1gm/kg protein. After breakfast, on the experimental morning, the bladder was emptied and this specimen discarded. Immediately there after 500cc of tapwater were administered orally. 60 minutes later 2cc of blood was withdrown, and hourly urines were collected exactly for 2 hours.
On the next morning the subjects were prepared same as mentioned above except oral administration of 4mg/kg Diamox at water loading. Hourly blood withdrawal three times and urine collection for 4 hours were done. During the test, subjects reclined quietly in bed except for voiding.
The subjects tested were all young male adults. Urine volume was measured in cubic centimeters and time was recorded in minutes. Creatinine determination was done with a photometer by Folin-Wu method.
Conclusion
1. The effect of Diamox on the kidney function at water loading was investigated by endogenous creatinine clearance method in normal adults.
2. The rate of water reabsorption was calculated approximately. In the diuresis induced by administration of Diamox, there was a decline in the rate of reabsorption of water.
3. The endogenous creatinine clearance was reduced caused by a decrease in urinary creatinine excretion and an increase in blood creatinine concentration. Hemoconcentration was confirmed by rountine hematologic method.
4. In a few cases of them, alkalinization of urine was seen in the 1st period of clearance and in the 3rd period it was seen in all cases.
